1. Agreement to Terms

Welcome to Care Hub Australia. By accessing this website or engaging our disability support, therapy, or home modification services, you acknowledge that you have read, understood, and agreed to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please discontinue use of this site and contact our team for assistance.

2. NDIS Service Agreements

All direct disability care and therapy supports are formally governed by an individualized NDIS Service Agreement executed between Care Hub Australia and the participant (or their legally appointed representative). In the event of any inconsistency between this website policy and a specific Service Agreement, the terms of the signed Service Agreement shall take precedence.

3. Participant Rights & Choice

Care Hub Australia is dedicated to upholding the NDIS Code of Conduct and the fundamental rights of people with disabilities. Participants are entitled to:

• Full choice and control over the services they receive and the workers allocated to them.
• Dignity, privacy, and freedom from violence, abuse, neglect, and exploitation.
• Involvement in all decisions regarding their care plans, goals, and schedule adjustments.
• Request a support worker re-matching or an advocate at any time without fear of penalty.

4. Fees & NDIS Price Limits

Care Hub Australia charges in strict accordance with the current NDIS Pricing Arrangements and Price Limits guide published by the National Disability Insurance Agency (NDIA). We do not charge above the gazetted price caps for supports delivered. Travel, provider travel allowances, and non-face-to-face report writing are itemized transparently in each participant's schedule of supports.

5. Cancellation & Short-Notice Policies

We understand that unexpected circumstances arise. To ensure fairness to our practitioners and respect their rostered time, cancellations must be communicated in accordance with standard NDIS rules:

• Allied Health Therapies: Minimum of 2 business days' notice for scheduled assessments or clinic appointments.
• Core Support Shifts: As stipulated in your Service Agreement and compliant with NDIS short-notice cancellation caps.

If notice is received within the required window, no cancellation fee applies.

6. Feedback & Complaints Process

Your feedback helps us continuously improve. We operate a fair, accessible, and transparent complaints management system. Participants or advocates may raise concerns directly by phone (1300 209 086) or email (info@carehubaustralia.com). If you feel a matter has not been resolved satisfactorily, you may contact the independent NDIS Quality and Safeguards Commission on 1800 035 544.

7. Website Content & Intellectual Property

All text, images, logos, graphics, and design assets published on this website are the intellectual property of Care Hub Australia Pty Ltd or used under legitimate licence. Reproduction or commercial exploitation without prior written consent is strictly prohibited.

8. Governing Law

These terms and conditions are governed by and construed in accordance with the laws of the State of Victoria and the Commonwealth of Australia.
