Care Hub started in Hadfield in 2021 to fix what families kept telling us was broken: too many workers, too little communication, and no idea what the plan budget was doing.

Hello, we're Care Hub.
Care Hub Australia was founded in 2021 in Hadfield, in Melbourne's north-west, after hearing the same story from family after family: the physio didn't know what the OT was doing, the support worker changed every fortnight, and nobody could explain where the plan budget went.
We built Care Hub to fix that. Support work and allied health under one roof, a single coordinator who knows your whole plan, small consistent teams, and a monthly progress note anyone can read. Today we support children, young people and adults across Melbourne, Parramatta and Greater Western Sydney.

Families in Melbourne's north-west told us the same three things: a different worker every fortnight, therapists who never compared notes, and no idea what the plan budget was actually doing.

Children, young people and adults with disability
Across metro Melbourne and Greater Western Sydney, in the home, at school and in the community. Every stage has different goals, so the support looks different too.
Children 0–8
Early intervention through speech, OT and psychology, in the home and at kinder or school. We work with your paediatrician and teachers, and help you through the early childhood pathway.

Therapy plus support work for school, friendships and independence at home. Consistent workers matter most at this age, so we keep the team small and steady.

Moving out, study, work and getting around on your own. Support work that teaches skills, and therapy that backs it up. You run the plan; we help you use it.

Melbourne's north-west and Western Sydney are home to families from everywhere, and so is our team. Tell us your language and we'll match a worker who speaks it where we can, and arrange a free interpreter through TIS National for every conversation where we can't.
