Before your plan's reassessment date, the NDIA will contact you to check in about how your plan is going and what should happen next. Coming prepared helps make sure your next plan reflects what you actually need.
1. Gather your evidence early
Ask your providers for progress reports a couple of months ahead. Good reports explain what supports were delivered, what changed, and what's recommended next, all linked to your goals. If you're with Care Hub, your monthly progress notes already do most of this work.
2. Look back at your goals
For each goal in your current plan, jot down: what progress you've made, what got in the way, and whether the goal still matters to you. It's fine to change goals as your life changes.
3. Think about what's changed
- Has your health, disability or living situation changed?
- Has a carer's situation changed?
- Did any part of your budget run out early, or go unused?
- Are there supports you needed but couldn't get?
4. Bring support to the meeting
You can bring a family member, friend, advocate or support coordinator. You can also ask for an interpreter. Write down the points you want to make so nothing is missed on the day.
5. After your new plan arrives
Read it carefully and check it matches what you discussed. If you disagree with a decision, you can ask the NDIA to review it. You generally need to ask within 3 months of receiving the decision.
